Wave 3 — Agent Review Layer

Introduce a shared issue model with rule, severity, confidence, evidence, and location.

Acceptance

  • Findings are stable JSON objects.
  • Stable identity is documented, either fingerprints or a clearly defined equivalent.
  • Existing raw evidence output remains available.
  • Tests cover serialization and ordering.
