Wave 3 — Agent Review Layer

Add agent-facing grouping for fix planning.

Acceptance

  • Grouping logic is single-source.
  • Tests cover representative findings.
  • Bucket names and semantics are documented.
  • Output does not imply auto-fix safety unless proven.
