Follow-up from PR #166 ralph review (STATUS-FSYNC-01).\n\n## Scope\nReplace write_text-based status artifact persistence with an explicit open/write/flush/fsync/close before atomic replace.\n\n## Acceptance criteria\n- Status artifacts are flushed and fsynced before replace.\n- Failure handling still records status_artifact_error and does not mask apply outcome.\n- Tests cover write failure behavior after the refactor.\n\n## Source\nPR #166 follow-up: STATUS-FSYNC-01.
