Pain
Signal, Matrix, webchat, subagents, Codex/ACP, and cousin-family rooms all produce relevant context. The dangerous failure mode is not “missing context”; it is silently merging sources and making Piotr's words, agent conclusions, and system artifacts look equivalent.

Without this, Iskra can answer “what happened?” but not “who actually said/approved this?” — and that is where trust breaks.
