Canary status: missing — fire 3+3 review before merge

Summary

Adds a read-only transition history surface for card.created and card.updated.list events.
Returns stable public IDs/names for cards, boards, fromList, and toList, plus source metadata.
Exposes the read path through Integration API and MCP tool get_transition_history.

Scope

agent.getTransitionHistory API endpoint.
KanIntegrationClient.getTransitionHistory and MCP registration.
Repository query over existing card_activity events.
Contract docs for analytics foundation.

Non-goals

No lead time, cycle time, throughput, WIP, CFD, dashboards, or mutations.
No backfill execution; that remains the explicit script from the previous PR.

Spec sources read

card_activity schema and list relations.
Existing getRecentActivity and MCP read tool patterns.
Kan-ductor working plan item analytics/event-foundation-v0.
