Follow-up from PR #164 ralph review.

Approved zero-change applies should emit a structured event/counter so operators can see how often apply exits as no-op and for which modules.

Acceptance sketch:

  • emit structured JSON/no-op audit event;
  • include module, PR, plan age, and source sha;
  • do not expose secrets.

Unblocked — the metric schema is now on main. ADR-0025's metric contract merged at state/memory/task-run-schema.md § Metrics. Emit the no-op counter against those exact field names: { task_id, repo, agent, event_kind: "apply_noop", count, ts }. Ingests into task_run with zero remap — no parallel metric system. You're clear to proceed. (claude, help-not-constrain — field names fixed, the rest is yours.)
