Audit feedback (claude orchestrator)

This issue's premise is wrong: INDEX.md exists in repo (175 lines, file dated 2026-04-30, status: "L4 complete, ready for hand-off to Codex GPT-5.5"). It is also referenced from README. The current README.md correctly links to it.

What IS true: INDEX.md is stale. The status line ("L4 complete, ready for hand-off to Codex GPT-5.5") was a point-in-time marker before Phases 02 v2 cataloging started, before ADRs landed, before all the recent governance work (PR #58 swarm-fundament, etc).

Required amendment before ready-for-agent

Change the issue body's premise from "INDEX.md does not exist" to "INDEX.md is stale (dated 2026-04-30, references pre-cataloging state)". Change the action from "create or deprecate" to one of:

  • (default) Refresh INDEX.md to reflect current state: list all top-level docs (PR #58 just made README the entry point; INDEX should now play the deeper-cross-reference role), recent ADRs, current Phase, link to most recent STATE_OF_PLATFORM.
  • (alternative) Deprecate INDEX.md if its role is now covered by README's mobile-scannable map. In that case, remove from PLATFORM_CHARTER §12 references too.

Recommendation in this issue body: pick (default — refresh) since PLATFORM_CHARTER.md §12 lists INDEX.md as a top-level doc, and removing it would require charter amendment, which is heavier.

The re-shaper (next agent or self-amend) should re-read README + AGENTS + PLATFORM_CHARTER §12 to understand the role split, then update INDEX.md accordingly.
