Canary status: missing — fire canary 3+3 manually before merge

Product story

Plan artifacts are the apply pipeline trust root. Their source_sha and dirty state must come from the actual git checkout containing the module manifest, not from an ambiguous caller working directory.

What changed

Resolved git toplevel with git rev-parse --show-toplevel before reading HEAD or dirty state, and anchored _new_plan() provenance to the manifest's containing checkout.

Why it changed

Issue #192 begins the provenance hardening segment after the redaction chain. It closes nested/wrong checkout ambiguity before plan-hash tamper evidence is added in #193.
