Follow-up from PR #164 ralph review.

Make missing/null exitCode a separately messaged malformed-plan error instead of relying only on membership checks.

Acceptance sketch:

  • explicit plan_exit is None branch;
  • test missing/null exitCode;
  • preserve current rejection behavior.
