fix(charter): repair 5 broken cross-references (charter consistency scan follow-up) 429980f695

Per `.sisyphus/research/charter-consistency-scan-2026-05-02.md` (GLM autoresearch
output): 10 broken references found, 5 fixable as small follow-up:

1. Line 289 (was: §4-pre-rollout) — §4 is Observability, identity-rules belong
   to §3 → "charter rule §3 service-identity sub-rule"

2. Line 295 (was: charter §4 v2) — same root cause → "charter §3 identity-isolation v2"

3. Line 524 (was: policies/tailscale-acl.hujson) — file lives at network/, not
   policies/ → "network/tailscale-acl.hujson"

4. Line 552 (was: state/L-1/baseline/) — baseline directory is at project root,
   not state/L-1/ → "baseline/ at project root"

5. Line 627 (was: entities) — actual file is entities.json, plus added .md
   extensions to JOURNEY/OPEN_LOOPS/CONTRADICTIONS/ORPHANS

NOT fixed in this PR (would need bigger scope change):
- §2 layout shows policies/ directory that doesn't exist (aspirational paths
  per scan recommendation #4 — needs §2 "materialized vs aspirational" note)
- 6 more aspirational-but-broken paths in §2 (will address when materializing
  the directories or marking them aspirational)

Net diff: +5 / -5 LOC (path replacements only, no policy change).

Per charter §3 (PR #22 amended): ordinary charter edits (typo fix, link repair,
single-clause clarification ≤30 LOC) qualify as **Small impact**. This PR is
pure path-fix, no prose change → push directly per operator pre-auth A.

Charter consistency scan summary post-fix:
- Total cross-references: 69
- Was OK: 42 → now 47
- Was broken: 10 → now 5 (5 remaining = aspirational §2 paths)
- Ambiguous: 5 (unchanged)
