Adds prompts/01.5c-pm-decision-packet.md per operator's architectural
correction (2026-05-01) of original 4-layer pipeline.

Original sketch: Codex → gates → GLM (1 reviewer) → Claude (1 PM-translator)
→ Operator. Operator pushed back: single PM-translator is single point of
failure, exactly the automation bias Oracle warned about.

Corrected pattern:

  • 3× technical reviewers (different model families: GLM, Claude, GPT) vote
    technical OK + list risks
  • 3× product reviewers (different framings: value, ADHD-aware, skeptic)
    vote product OK + list opportunities/risks
  • Deterministic consolidator (Python, no AI judgment) aggregates votes
  • All-6-OK = candidate for operator approval
  • Any dissent = surfaces in decision packet with default action 'defer'

Plus:

  • state/STATUS_NOW.md initialized for Dispatch monitoring
  • state/AUDIT_LOG.jsonl entries for PR #1 merge + this session

in §8 of the spec.
